A lot of times our emotions, either nervousness or anger, can get in the way and we tend to be short and sweet and not explain where we are coming from. Oftentimes offense is created from not explaining your point of view properly. Having the door open for dialogue (back and forth conversation between two people) and being patient in your conversation can reduce the chance of offense. Oftentimes conversations will look completely different than we anticipate if we just take the time to listen.
